Black Men's Health National Survey

Father's Day is a day to honor the many men who have the role of being a father; be it biological, surrogate or in-law. Fatherhood is truly one of the most awesome leadership positions to hold; their mentorship, wisdom, and unconditional love is the glue that binds the family unit. 

We need our Black fathers and often times they have unique health needs and challenges that shorten their life-span. In the spirit of this day, we ask that all Black Men take the Black Men's Health Survey. This survey collects health information and behavior to better address the unique health care needs and challenges of Black men. The survey link is below.
